Purpose:
Allows one to name an agent to assume control of their remains upon death.
Notes:
- This allows one to state their burial wishes upon death (i.e. cremation, location of burial, embalmment).
- Allows for alternate agents to be named in the event the original agent is not able to perform the stated duties.
- Should provide copies to listed agent, alternates and any businesses performing burial duties (i.e. cemetery organization, crematory, funeral director).
- Requires notarization.
