Purpose:
Names the person you wish to manage your affairs in the event that you lose mental capacity.
Notes:
- Allows you to name up to 4 designees (1 guardian and 3 alternates in order of preference).
- Allows you to name persons you would like to disqualify from serving as guardian.
- Addresses guardianship of both person (medical) and estate (financial).
- Requires 2 witnesses and notarization.
