1. A HealthBridge Care Manager contacts your patient and schedules a initial telephone consultation.
2. Then, one of HealthBridge's Certified Benefits Counselors meets your patient for a free, no obligation care planning session. 
   
3. Once they've determined the best plan of care, HealthBridge selects a few caregivers for your patient to interview.
4. We'll keep in touch with you to ensure you're in the loop with your patient's care. 
   
5. That's it!  We take care of everything else.
