Post Rehab Care provides support and assistance after your loved one is discharged from a rehabilitation hospital. When your loved one returns home from rehab, they will likely find themselves with in home or outpatient physical, occupational and speech therapy. They will also have several follow up appointments to their primary care doctor and specialists. Post rehab care can include:
- A plan of care specially designed to help people who have home therapy.
- Assistance with home exercise program
- Help with activities of daily living like bathing, dressing and grooming
- Transportation to outpatient rehab appointments
- Transportation to follow up appointments with orthopedists and primary care physicians
- General help and assistance while your loved one is regaining strength.
- Your caregiver will perform all of the tasks found in Caregiver Services or Live In Care, depending on your needs.
