Your employer will also provide information to HealthBridge, which includes, but is not limited to, employer name, plan participant and dependent names, length of employment, annual income, addresses, dates of birth, email addresses, health plan eligibility, and other relevant information needed to verify the identity and employment of plan participants and their dependents. Your health insurer (or health plan administrator) will also provide information to HealthBridge about health care treatment you or your family receive and health care service providers you receive treatment from. All of this information is protected under the Health Insurance Portability and Accountability Act (“HIPAA”). As a Business Associate of your group health plan or insurer, HealthBridge is required to protect all information defined as personal health information, or PHI, under HIPAA. We also may collect aggregate information, which is data we collect about your use of our or third-party websites or service from which personal information has been removed. We use this aggregated data to help us understand consumer trends, needs, interests and preferences so we can improve our products and services. HealthBridge also collects demographic information, which is anonymous and may include:

If your visit to our website is limited to browsing our informational content, we will not require that you provide us with any personally identifiable information. However, HealthBridge will collect information about your computer or mobile device to improve and evaluate your use of our website. HealthBridge takes the privacy and security of its members’ personal information seriously. We maintain administrative technical and physical safeguards to protect the security, confidentiality and integrity of your information.

We protect personal information you provide online when you register yourself as a user of our website. Access to your own (and your dependents) personal information is available through a unique user ID and password you select. This password is encrypted while transmitted from your browser to our servers and while stored in our systems. To protect your personal information, never share your password with anyone. Your saved personal information is stored on servers that only selected HealthBridge employees and vendors have access to. We encrypt all data that you submit through www.myhealthbridge.com during transmission using Transport Layer Security (TLS) to prevent unauthorized parties from viewing your information. All information you submit to HealthBridge by email is not secure, so please do not send sensitive information in any email to us. We never request that you submit sensitive or personal information over email, so please report such requests by sending an email to privacy@myhealthbridge.com.

How do we use your information?

Our primary use of your information is to administer, maintain and improve your experience on our Web Site generally as well as provide you with customized, personalization services and interactive communications.
